Use this Order Form to register a BMM Member when you will use a School Purchase Order or School Credit Card. The Participant should be finishing a recent course and continuing with BMM Membership.

***IMPORTANT*** Participant accounts are created based on the email addresses you type on this form. PLEASE be accurate. If you are adding a participant who has taken a Build Math Minds Course in the past please use the same email address the course was purchased under. This will put all the Participants information in one account.

    This information should be the person responsible for purchase order processing/payment. The invoice will be sent to the email address entered here. The participants information will be entered later. This person has school authority to commit funds for payment to Build Math Minds.
  • We understand each District/School may have their own purchase order processes. Please choose the option below that best describes your School Purchase procedure so that we can process your transaction correctly.

    If you do not attache a PO during this registration and need to send later, please email it to: If you choose "Pending" we will allow 30 days for submission. After 30 days a "Pending" registration will be suspended until a PO or payment is submitted.
